Clinical Care Coordinator

Summary:
An exciting opportunity exists for a compassionate and experienced Clinical Care Coordinator to join a values-driven aged care provider. In this pivotal role, you will work closely with the Facility Manager and wider clinical team to ensure the delivery of high-quality, person-centred care to residents. This is an ideal opportunity for an experienced Clinical Coordinator or Clinical Manager ready for a fresh challenge in a supportive environment.

Responsibilities:
  • Support the Care Manager in overseeing clinical governance and daily care operations.
  • Lead and mentor the care and nursing team to ensure consistent, high-quality care delivery.
  • Conduct clinical assessments, care planning, and ongoing review of resident care.
  • Monitor and support compliance with Aged Care Quality Standards and relevant legislation.
  • Coordinate clinical documentation and ensure accurate and timely reporting.
  • Collaborate with allied health professionals, residents, and families to enhance resident outcomes.
  • Assist in staff education, onboarding, and performance management.
Key Requirements:
  • Registered Nurse with AHPRA Registration.
  • Previous experience in a similar clinical leadership role within residential aged care ( ESSENTIAL).
  • Strong knowledge of Aged Care Quality Standards and clinical best practices.
  • Excellent interpersonal, communication, and leadership skills.
  • Ability to work collaboratively with multidisciplinary teams.
  • Sound computer literacy and proficiency in clinical documentation systems.
  • Compassionate, professional, and resident-focused approach to care.
