Clinical Care Coordinator

Summary:
As Clinical Care Coordinator, you’ll play a vital role in ensuring safe, high-quality care delivery. Reporting to the Clinical Care Manager, you’ll support day-to-day clinical operations, lead a dedicated team of nurses and carers, and help shape a culture centred on dignity, inclusion, and continuous improvement. This is an ideal opportunity for an experienced Clinical Coordinator or experienced Registered Nurse ready for a fresh challenge in a supportive environment.

Responsibilities:
  • Support the Clinical Care Manager in overseeing clinical governance and operational excellence.
  • Provide hands-on leadership to the nursing and care teams, ensuring safe and effective care.
  • Conduct and oversee clinical assessments, care plans, and ongoing care reviews.
  • Champion compliance with Aged Care Quality Standards and internal policies.
  • Ensure timely, accurate clinical documentation and reporting.
  • Liaise with allied health professionals, GPs, residents, and families to promote optimal outcomes.
  • Contribute to onboarding, training, and performance development of care staff.
  • Participate in audits, accreditation preparation, and quality improvement initiatives.
Key Requirements:
  • Registered Nurse with current AHPRA registration (Essential).
  • Demonstrated experience in a clinical leadership role in residential aged care (Essential).
  • Strong knowledge of Aged Care Quality Standards, clinical governance, and best practices.
  • A team-oriented leadership style with excellent mentoring and coaching abilities.
  • Outstanding interpersonal and communication skills.
  • Confident in documentation, clinical software systems, and care planning tools.
  • A compassionate, resident-first approach with a commitment to continuous improvement.
