Clinical Care Coordinator – Short-Term Contract

About the Role:
We have a short-term (8-week) opportunity for a compassionate and experienced Clinical Care Coordinator to join a friendly and supportive aged care team. Working closely with the Facility Manager and wider clinical staff, you’ll play a key role in ensuring residents receive exceptional, person-centred care. This is a perfect opportunity for an experienced Clinical Coordinator or Clinical Manager looking for a fresh, rewarding challenge in a positive environment.

Your Responsibilities:
  • Support the Care Manager in overseeing daily care operations and clinical governance.
  • Lead, mentor, and inspire the nursing and care team to deliver high-quality care.
  • Conduct clinical assessments, develop care plans, and review resident care regularly.
  • Monitor compliance with Aged Care Quality Standards and relevant legislation.
  • Ensure accurate clinical documentation and timely reporting.
  • Collaborate with allied health professionals, residents, and families to achieve the best outcomes.
  • Assist in staff onboarding, education, and performance management.
Key Requirements:
  • Registered Nurse with current AHPRA registration.
  • Previous experience in a similar clinical leadership role within residential aged care (ESSENTIAL).
  • Strong knowledge of Aged Care Quality Standards and clinical best practices.
  • Excellent interpersonal, communication, and leadership skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Sound computer literacy and experience with clinical documentation systems.
  • A warm, compassionate, and resident-focused approach to care.
  • Available to start ASAP.
