Registered Nurse – Home Care

We are seeking a passionate and dedicated Registered Nurse to join a leading not-for-profit aged care provider in Ballarat. You will deliver high-quality clinical care and support to clients in their homes, promoting independence, wellbeing, and a better quality of life.
Key Responsibilities
- Conduct comprehensive health assessments and develop personalised care plans for clients.
- Administer medications, provide wound care, and monitor clients’ health conditions.
- Collaborate with clients, families, and multidisciplinary teams to deliver holistic care.
- Educate and support clients and families to promote health and wellbeing.
- Maintain accurate, up-to-date clinical documentation in line with Aged Care Quality Standards.
- Identify and manage risks, escalating issues when necessary.
- Contribute to continuous improvement initiatives within the Home Care service.
- Current AHPRA registration as a Registered Nurse.
- Experience in Community Care or Home Care settings.
- Endorsement to conduct Continence Assessments and prescribe continence aids (highly desirable).
- Proven ability to assess, implement, and evaluate individualised care and support plans.
- Strong leadership skills, with the ability to guide and mentor others.
- Experience educating clients, families, and care staff.
- Proficient in using the My Aged Care portal.
- Registered and insured personal vehicle.
