Summary Of Position
An exciting opportunity exists for a passionate and dedicated Registered Nurse to join a leading not-for-profit aged care provider in Ballarat. In this role, you will provide high-quality clinical care and support to clients within their homes, promoting independence and enhancing their quality of life.

Key Responsibilities
  • Conduct comprehensive health assessments and develop personalised care plans for clients.
  • Administer medications, provide wound care, and monitor clients' health conditions.
  • Collaborate with clients, families, and multidisciplinary teams to deliver holistic care.
  • Provide education and support to clients and their families to promote health and wellbeing.
  • Ensure all clinical documentation is accurate, up-to-date, and compliant with Aged Care Quality Standards.
  • Identify and manage risks, escalating issues where necessary.
  • Contribute to continuous improvement initiatives within the Home Care service.
Requirements
  • Current AHPRA registration as a Registered Nurse.
  • Experience as a Registered Nurse within Community Care or Home Care.
  • Endorsed to conduct Continence Assessments and prescribe continence aids (highly desirable).
  • Proven ability to assess, implement, and evaluate individualised care and support plans.
  • Strong leadership skills with the ability to guide and mentor others.
  • Demonstrated experience in educating clients, families, and care staff.
  • Proficient in using the My Aged Care portal.
  • Must have a registered and insured personal vehicle.
