Are you an experienced Case Manager looking to step into a more senior, impactful role? This is your opportunity to join a values-driven aged care organisation, supporting people living at home with dementia and other complex needs. You’ll handle a smaller caseload of high-needs clients while mentoring and guiding a team of Case Managers.

What You'll Do:

• Manage a portfolio of complex Home Care Package (HCP) clients
• Support onboarding, coaching, and ongoing development of other Case Managers
• Act as a senior point of contact for escalation and problem-solving
• Collaborate with internal clinical, finance and rostering teams to ensure seamless care delivery
• Uphold quality, compliance, and best-practice service coordination under Aged Care Quality Standards

• Proven experience in case management within the aged care or community care sector
• Confident in managing complex client needs and navigating diverse situations
• Enjoy mentoring and supporting team development
• In-depth knowledge of HCP funding and consumer-directed care models
• A clinical background (RN, EN or Allied Health) is desirable but not essential
• Compassionate, organised and solutions-focused
